All children enrolling in kindergarten in a public or private school shall have an eye examination performed by a physician, or licensed optometrist. 

This exam is to be completed within the previous one year prior to the child entering an Illinois School for the first time. 

For students entering an Illinois School from out of state or out of country the exam is required to be submitted. 

A waiver for children who show undue burden or lack of access may be presented in place of an exam.
