IHSA (Illinois High School Association) rules require student-athletes to have a current, signed physical on file with their school before participating in any practice or contest.

Physicals are valid for 395 days from the date of the exam.

We follow this rule for all athletes in grades 6-12.

They must be performed by a licensed physician, physician assistant, or nurse practitioner.

The sports physical form does not fulfill the school health exam for students in grades 6 & 9.
